Fees and Insurance

I participate with CareFirst BCBS Federal Employee Program and PPO insurance. As a participating provider, I am responsible for filing the claim with CareFirst BCBS. Your co-pay is due at the time of service.

In cases where clients have coverage with another insurance company, I will bill you monthly. The statement I prepare can be submitted to your insurance company for reimbursement. Payment may be made with cash or by check.

Cancellation Policy

Your appointment time is reserved in advance for you in order to ensure the continuity of our therapeutic work. Therefore, there is a charge for cancelled and broken appointments with the exception of true emergencies or illness. The charge is the full fee for the appointment. In cases of illness or emergency, I will make every effort to arrange an alternate appointment time if possible.

Confidentiality

As a client, you possess the right to confidentiality concerning all information about your care. There may be occasions when you and I agree that it would be helpful to your care for me to confer with another treating party. In that event, I would have you sign a Release of Records before contacting that person. There are occasions when I must violate your confidentiality, however: if I believe you are in imminent physical danger; if I believe you pose an imminent physical threat to another; or if I have reason to believe a minor or vulnerable adult is being or has been abused.

Communication

I recommend communication by telephone for the greatest privacy between us. I prefer to restrict the use of email to communicating between sessions about scheduling changes, or for the exchange of forms. I respond to phone messages and emails within 24 business hours.

Forms

I need two forms filled out by our first appointment. You may download them here and email scanned completed versions to me, or bring them with you if we are meeting in person.
